Registration Information
Register for the 2003 SICB Annual Meeting via SICB's secure Web site or use the form enclosed in the registration brochure (downloadable as PDF file via this link). Make sure all sections of the conference registration form are complete, and proper payment is included to avoid processing delays. Individuals registering as non-members will automatically receive SICB membership for 2003, simply by completing the application form.

Three Ways to Register
To qualify for discounted registration fees, registration forms and payment must be received at the SICB Business Office by Friday, December 10, 2002.
- Register online using SICB's secure website
- Register by fax: Fax your completed form with credit card information to (703) 790-2672. VISA, Mastercard, Discover and American Express accepted.
- Mail your completed form (payable to SICB) with payment to:

Mail completed registration form with check, money order, purchase order or credit card information.
Registration Fee Categories
Most non-member categories include SICB membership for 2003. If you are not already a SICB member, fill out the membership application/dues Renewal form to receive your complimentary membership. If you are a member, use that form to pay your 2003 dues.

Registration Cancellation Policy
All cancellations are subject to a $50 service charge. Cancellations must be in writing via fax, email or regular mail to the SICB Business Office. Cancellation notices received by December 10 will be refunded total registration fees minus the $50 service charge and will be refunded after the meeting. No refunds will be issued on cancellations received after December 10.

Confirmation letters will be sent once payment has been received.
